Professional Referral
For professionals and institutional referrers — GPs, social workers, schools, probation officers, and other agencies — to refer an individual through a professional channel.
Priority handling: Professional referrals are reviewed as a priority. You may be contacted for additional information to support the referral. Submitting a professional referral does not automatically enrol anyone into the SafeCheck programme — engagement remains entirely voluntary.
Do not include the individual's name or identifying details in this form. If your referral leads to an outreach, identifying information will be collected through the programme's intake process directly with the individual.
