If you are concerned about someone you know, you can submit a third-party referral.
Important: A third-party referral does not enrol an individual into SafeCheck. It triggers a review and an outreach attempt only. Engagement with the programme remains entirely voluntary. You will not be given information about whether the individual participates or their programme status.
Do not include the individual's name or identifying details in this form. If your referral leads to an outreach, identifying information will be collected through the programme's intake process directly with the individual.
